How to Apply for the SRC Life Insurance Policy at University of Cape Coast (UCC)

Step-by-Step Application Process

1. Present Evidence of Death Certificate of Guardian/Parent

✍️If you are applying for the life insurance policy due to the unfortunate loss of a parent or guardian, you must submit a valid death certificate as proof.

2. Submit the Medical Cause of Death

✍️Along with the death certificate, applicants are required to provide the medical cause of death as outlined by a medical practitioner or health institution.

3. In Case of Disability Instead of Death

✍️For cases involving the disability of a guardian or parent (rather than death), applicants must present evidence of the disability. This could include medical documentation or certifications confirming the condition.

4. Attach a Letter of Request

✍️Applicants must attach a formal letter requesting the insurance claim. The letter should be addressed to the SRC President, clearly stating the reason for the claim.

5. Provide a Screenshot of School Fees Payment

✍️A screenshot showing proof of payment for school fees must also be included as part of the documentation.

All the required documents should be submitted to the SRC Office, located behind Valco Hall.
